You are correct - it sticks out and you cannot close the car door while the device is in the door jamb slot. The handle can be placed in the "slot" two ways in our car - one where the round-end of the handle is pointing outside and the other way where the round-end of the handle is pointing to the inside door hinges. We use both ways depending on whether our parent is getting in or out of the car. But once the handle is no longer being used, we remove it from the "slot" and place it in the door pocket (or glove box) until the next time it is needed. Hope this helps.

Q: Will it work in a toyota sienna

Look at the Door Post and see if there is a U shape piece that extends from the column that the latch of the door attaches to when the door is shut. You insert the metal "beak" into the center of that and the red part then extends out to be the temporary handle. You can either set it in so that the handle extends across the door opening or so that it sticks out from the car. You remove the handle each time you get in and out of the car (so plan to store it in a door pocket or similar location). I have not yet seen a car, pick-up or van that it would not work in but I am not familiar with the Toyota Sienna. I have used mine in a Subaru and a Honda.

Like other replies, you can use the Handy Bar either way , the handle pointing away from the vehicle or pointing across in front of you. Don't know about the Car Cane, not sure why you asked about that one, since that's not the one I bought. Hopefully it will be equally helpful for you. :)
